The goal of the Clery Act is to provide information to the campus community to promote crime awareness and to enhance campus safety through providing reliable statistical records. This report form provides a uniform method to document reportable crimes and/or non-criminal hate motivated incidents that have occurred on and around Washington University Clery reportable geography, which have been reported to a Campus Security Authority (CSA) other than the Washington University Police Department. CSA’s are defined as someone who has significant responsibility for student and campus activities. The information collected is not intended to be used to identify the victim, but rather to meet CLERY ACT requirements and to be used to increase public safety. CLERY documentation is not satisfied by simply directing/referring the reporting party to the police department. In order for the University to satisfy the statistical reporting requirements of the Clery Act, all CSA’s are required to complete this form when offenses are reported to them. First Name Last Name Email Phone Incident / Offense Type Aggravated Assault Unlawful attack upon another with intent to inflict sever injury or great bodily harm Arson Willful or malicious burning / attempt to burn structure; vehicle; or personal property of another Burglary Unlawful entry into a structure to commit a felony or theft. Dating Violence Violence committed by a person who is or has been in a social relationship of a romantic or intimate nature with the victim. Dating violence includes but is not limited to sexual or physical abuse or the threat of such abuse. Dating violence does not include acts covered under the definition of domestic violence. Destruction/Damage/Vandalism of Property To willfully or maliciously destroy, damage, deface, or otherwise injure real or personal property without the consent of the owner or the person having custody or control of it (only include in the instance of a Hate Crime where the offense was committed based on a bias against the victim) Domestic Violence Felony or misdemeanor crimes of violence committed by a current or former spouse or intimate partner of the victim, by a person with whom the victim shares a child in common, by a person who is cohabitating with or has cohabitated with the victim as a spouse or intimate partner, by a person similarly situated to a spouse of the victim under the domestic or family violence laws of the jurisdiction receiving grant monies, or by any other person against an adult or youth victim who is protected from that person’s acts under the domestic or family violence laws of the jurisdiction. Drug/Narcotic Violations possession or distribution of controlled substances or possession of drug paraphernalia Homicide Non-negligent manslaughter; and negligent manslaughter including vehicular manslaughter Intimidation Unlawfully placing another person in reasonable fear of bodily harm through the use of threatening words and/or other conduct; but without displaying a weapon or subjecting the victim to actual physical attack (only include in the instance of a Hate Crime where the offense was committed based on a bias against the victim) Larceny-Theft Unlawful taking, carrying, leading or riding away of property from the possession or constructive possession of another (only include in the instance of a Hate Crime where the offense was committed based on a bias against the victim) Liquor Violations underage possession or consumption; distribution to minors Motor Vehicle Theft Theft of automobiles; trucks; etc.; including ‘joyriding’ (taking by person without lawful access) Robbery Taking or attempting to take something by force; violence; threat; or by putting victim in fear Sex Offense (Forcible) Rape; sodomy; sexual fondling; or sexual assault with object Sex Offense (Non-forcible) Statutory rape (victim under the legal age of consent) or incest Simple Assault Unlawful physical attack by one person upon another where neither the offender displays a weapon, nor the victim suffers obvious severe or aggravated bodily injury involving apparent broken bones, loss of teeth, possibler internal injury, severe laceration, or loss of consciousness (only include in the instance of a Hate Crime where the offense was committed based on a bias against the victim) Stalking Engaging in a course of conduct directed at a specific person that would cause a reasonable person to fear for his or her safety or the safety of others, or suffer substantial emotional distress.
