See below for details and see Habif’s Mission Statement and policies regarding non-discrimination and equal visitation: Patient Rights As a patient, you have the right to: receive courteous, considerate care be treated with respect privacy and confidentiality regarding your treatment and medical records review your medical records, knowing they remain the property of Habif be informed of the effectiveness of treatment, and possible risks, side effects, or alternate methods of treatment have the choice of a clinician and to change clinicians if desired know who is treating you refuse treatment, ask for a second opinion and/or an alternative course of treatment, and to be informed of the medical consequences of your actions be informed of personal responsibilities involved in seeking medical treatment and maintaining health and well-being after treatment bring any dissatisfaction to the attention of the attending health professional and/or their supervisor Patient Responsibilities As a patient, you are responsible for: presenting accurate identifying information. This includes information about all aspects of your health condition, any medications — including over-the-counter products and dietary supplements, and any allergies or sensitivities presenting details of illness or complaint in a direct and straightforward manner cooperating responsibly with all persons involved in the health care process attending your appointments on time canceling appointments only when absolutely necessary, and far enough in advance so that other patients might utilize that time paying for all services rendered, including any charges not covered by health insurance complying with the treatment plan provided by the health professional asking for clarification when you do not understand information or instructions providing both positive and negative feedback to the health professional(s) responsible for your care showing respect to all health care professionals and staff, as well as other patients providing a responsible adult to transport you home from the facility and to stay with you as needed, if required by the provider informing your healthcare provider of any living will, medical power of attorney, or other directive that could affect care Related Resources HIPAA Privacy Policy and Procedures Learn about Washington University's policies and procedures related to the HIPAA privacy and security rule.
